چکیده

OBJECTIVES/HYPOTHESIS Our objective was to review our experience with a conservative central boat harvest of costal cartilage in patients undergoing rhinoplasty. It involves taking only the central portion of the rib for reconstruction. When cartilage harvest is performed in the above manner, donor site morbidity is minimized without limiting aesthetic results. The key seems to be preservation of intact costal cartilage on three sides, limiting harvest to the central portion only. This central portion is straight and much less prone to warping than the cartilage toward the periphery. STUDY DESIGN A retrospective review of a single surgeon's experience. METHODS All rhinoplasty operations performed by the senior author (Y.D.) from January 2000 to August 2009 that required the harvest of rib cartilage were reviewed. RESULTS A total of 322 cases were identified in which rib cartilage was harvested via the described technique. In all cases sufficient cartilage volume was obtained for the intended purpose. Average operation time was 10 minutes. No drains were used, and only one patient developed seroma formation. Postoperative pain and scar were minimal. No major complications were noted, and on postoperative analysis there was no evidence of cartilage warping or displacement. CONCLUSIONS When harvesting costal cartilage for rhinoplasty, the above technique allows for sufficient graft tissue while decreasing donor site morbidity and minimizing warping.

10th Rib Cartilage: Another Option of the Costal Cartilage Graft for Rhinoplasty

The septal and auricular cartilages have traditionally been considered primary autologous graft materials for rhinoplasty [1-6]. However, the autologous costal cartilage can be useful when a strong strut is required due to contracture from previous surgeries or accidents, when tip projection is required, when the septal and auricular cartilage cannot be used due to a previous surgery, or when a...

Complications Associated With the Use of Autologous Costal Cartilage in Rhinoplasty: A Systematic Review.

BACKGROUND Autologous costal cartilage grafts are common in rhinoplasty. To date, no formal systematic review of complications associated with autologous costal cartilage grafting in rhinoplasty exists. OBJECTIVES The authors review current literature to examine the rates of donor and recipient site complications associated with autologous costal cartilage in rhinoplasty. METHODS Databases ...

Ultrasonographic evaluation of calcification patterns in costal cartilage: implications for rib graft harvesting.

IMPORTANCE Complex augmentation rhinoplasty often requires the use of cartilaginous grafts, especially in revision surgery. When using costal cartilage, the possibility of inadequate cartilage material because of excessive calcification must always be kept in mind. Furthermore, cartilage may be harvested but found to be not ideal, causing unsatisfying results. OBJECTIVE To report our experien...

Outcomes following rhinoplasty using autologous costal cartilage.

OBJECTIVE To describe the aesthetic and clinical outcomes following rhinoplasty using autologous costal cartilage, which is considered the best graft material for rhinoplasty requiring major reconstruction. Few studies have examined outcomes following rhinoplasty using autologous costal cartilage. METHODS A retrospective review of the data from 108 patients who underwent rhinoplasty using aut...

Avoiding pitfalls in open augmentation rhinoplasty with autologous L-shaped costal cartilage strut grafts for saddle nose collapse due to autoimmune disease: the Cambridge experience.

INTRODUCTION Saddle nose deformity due to autoimmune diseases such as Wegener's Granulomatosis and Relapsing Polychondritis is aesthetically, functionally and psychologically distressing for patients. However, "reliable" options for surgical correction remain limited in the literature. We present our experience of augmentation rhinoplasty in this patient population focussing on the techniques a...
